Requirements for Proxy SIM Swap Service.

NB:
• A customer can only authorize a proxy swap request on their line
• Customer and Proxy NIN must not be BVN generated. Where NIN is BVN generated, the customer and Proxy must have visited NIMC enrollment center to update NIN details and make NIN active before requesting for Proxy Swap. Proxy Request will not be processed when NIN is BVN Generated.

1. Letter of authorization, authorizing MTN to perform swap by Proxy – Letter must state Reason for the swap and unavailability and must be duly signed
2. Letter must state Customer and Proxy details: -
Customer details :-
• Customer Name
• Phone number to be swapped
• Virtual NIN (mandatory)
• 3 Frequently called numbers – 3 numbers frequently called by the phone number to be swapped
• Last Airtime Recharge Amount.
• Last Data bundle purchased
• Year of SIM/Phone Number Activation

Proxy details: -
• Proxy Name
• Proxy Address
• Proxy phone number- (NB: Proxy phone number must be an existing MTN number active for more than 30 days).
• Proxy Virtual NIN

3. Indemnity letter - indemnifying MTN of any outcome of Proxy swap, Letter should be signed by the customer and Proxy- (This can be a one line paragraph on authorization letter and where this is the case, both customer and proxy must sign the authorization letter)
4. NIN Update Consent: Giving MTN consent to update SIM Registration records with his/her NIN records. This can also be a one-line paragraph on the authorization letter.
5. Virtual NIN of both customer and Proxy – where the customer is outside the country, he / she shall also provide also a copy of his / her International passport data page, international resident permit or international driver’s license.
6. Proof Ownership - SIM Pack/SIM Certificate / or Affidavit from Court or Notary Public in the absence of a SIM Pack or SIM Certificate. Affidavit / Notary must state the customer name and NIN Number and must clearly state that customer is the owner of line 0803xxxxxxxxx.
7. Affidavit for Reason for Proxy Swap: Customer swears an affidavit in a court / notary detailing why he is unable to visit in person swap. Affidavit letter must clearly state the Customer details (Name, Virtual NIN and Phone Number) and Proxy Details (Name, Virtual NIN and Phone number), Reason why customer is unable to visit for swap and Customer has authorized and appointed proxy to carry out swap on his behalf.
Affidavit for Proof of ownership (Item 5) and Reason for Proxy Swap (Item 6) can be on same letter and must be affixed with customer passport. Affidavit must only be sworn by the owner of the line.
8. Proxy should indicate the shop to be visited- date and time of visit, so as to notify the store representatives of the visit. Customer to Provide same documents to Proxy. Proxy shall visit store with the hardcopies of listed documents above for processing.
Note: A Proxy is eligible to stand-in as Proxy where
• Your Proxy must be an existing MTN customer for more than 30days and with a valid National Identification Number (NIN)
• Your Proxy must not have stood in as a proxy for another customer within the last 90 days.
• Both Customer and Proxy NIN is not BVN generated

Furthermore, your line must have been successfully linked to your NIN for the request to be treated.

Where NIN is not linked, you are advised to please submit and link your NIN via mtn.ng using URL, https://nin.mtn.ng/nin/ . You can enter your preferred email address to receive OTP and also give consent before submission.

Your Virtual NIN is required to link your NIN.

The virtual NIN can be generated via the NIMC mobile app or by dialing *346*3*11-Digit-NIN*109071# on any active MTN line.
A service charge of N20 applies upon dialing the USSD code. The VNIN can only be used once and expires in 72hours used or not used.
